The FG Approach Requires a Long-Term Teatment Program
FGI is a fully telemedicine practice. Vaginal fluid testing is conducting using at-home collection kits that are mailed back to our in-house laboratory for analysis by Dr. Lea. Appointments are conducted over Zoom calling or via audio-only for patients with limited access to technology. After initial evaluation, patients can expect ot reqiure multiple follow-ups as often as every 4 months over the course of treatment to monitor progress and optimize treatment. This can amount to consultation fees totaling estimated $2000-$3000 over the course of 12-24 months and before transitioning from active treatment to annual maintenance treatment. During the course of treatment, Dr. Lea is available by phone and email to counsel patients for not only complications and/or challenges with their treatment program but also to provide encouragement. Fowler Gyn International provides expertise for chronic vulvovaginal conditions for which patients have not been able to find relief and that have proven too challenging for most gynecologists to identify an effective and sustainable solution. It is wishful thinking for a quick solution, but the reality is in order to treat root causes, FGI asks of patients to make the commitment to themselves to persist with treatment during periods of doubt and discouragement. FGI is a SELF PAY Outpatient Medical Practice
FGI dose not accept or participate in any health insurance plans. Patients pay for services at the time service is rendered except when otherwise specified. Payment can be made with credit card or debit card. Providing insurance information is not necessary nor will any insurance company be billed for services you receive from FGI. However, if you are eligible and upon request, FGI will provide a Health Insurance Claim Form (Superbill) to submit to your insurance company for out-of-network coverage. Alternatively, these expenses may qualify for Flex Medical Spending Account or Medical Savings Account; check with your policy administrator.
If you are a Medicare beneficiary, FGI does NOT accept or participate in Medicare. Payments for services from FGI are paid directly by you without submitting a Medicare claim or Health Insurance Claim Form (Superbill).
New Patient/Initial Evaluation: $620
$50 Registration fee: Goes toward total cost of evaulation.
Zoom call (15min) to review your medical history, learn what problems you are dealing with, screen for contraindications to treatment, and determine if you are a good candidate for Dr. Lea's treatment programs. If it is decided not to proceed with initial evaluation, the $50 registration fee is refunded.
At-home collection: Shipped to your designated address and includes a prepaid return label and instructions to prepare for self-collection of your vaginal fluid sample.
Return of sample: When test results are ready, you are contacted via email to schedule your virtual appointment with Dr. Lea.
Zoom call: Review test results, review medical history again, and decide treatment plan. Oustanding balance for evaluation fee is paid at conclusion of visit. A health insurance claim form (Superbill) can be requested. Any prescriptions are faxed directly to the pharmacy. A copy of your visit documentation is sent to you via email.

Associated Costs
Prescription medications: Can include custom compound medications made by custom compounding pharmacies and commercial prescription medication available at regular pharmacies (ie Walgreens, CVS, Walmart). Most all medications are for local vaginal or vulvar use ONLY. Custom compounds cost approximately $80-$130 for 90-days-supply. Commercial medications are usually covered by patients' regular health insurance and patients pay copays per their benefits plan.
Over-the-counter products: Dr. Lea recommends adoption of specified skin care products and hygiene habits during the course of treatment. These products are commercially available (Walgreens, CVS, Walmart, Amazon), and their prices are comparable to those of household brands (shampoo, body soap, body lotion).
