All Prospective New Patients must:
- Be at least 18 years or older. Contact Dr. Lea if you seek an exception.
- Not be pregnant or nursing
- Have access to internet and email
- Have a mailing address
- Be willing to conduct consultations and treatment via telehealth
WHY "PROSPECTIVE" PATIENT? Because you may or may not be accepted for the services at FGI. On review of the information you provide, if there are factors that cause concern or warrant a pelvic exam or consultation with another type of specialist, you will be notified and your payment for registration will be refunded in full.
HOW LONG DOES IT TAKE TO DO THIS REGISTRATION YOU ARE ABOUT TO START? 10-12 minutes. Use a laptop or desktop computer. Do not try to it on your phone.
DO I SCHEDULE MY APPOINTMENT ONLINE? NO, the the appointment date is not established during the registration process. After your questionnaires information is submitted and reviewed, you will receive an email with a link to schedule a 15 minutes appointment for prospective patients via Zoom. The purpose of this call is for Dr. Lea to clarify any details, determine if evaluation with her is warranted, and determine if you are a good candidate for the treatment programs offered. Once the decision is agreed to move forward with the new patient evaluation, you will receive via UPS an at-home collection kit in order to perform vaginal fluid testing. After you return your specimens and your results are ready, you will receive via email a link to schedule your new patient appointment (60 minutes) via Zoom video.
WHAT TO EXPECT? You will be prompted to create a user account with ID and password that will be linked to your email for FGI communications. Then you will make a registration payment of $50.00 by credit card. During this registration, you will complete 1-2 vaginal symptom questionnaires and the General Medical Questionnaire. It takes about about 10-12 minutes to complete. You must do it in one session. You CANNOT pause and return to finish later. Please allow up to 7 business days for Dr. Lea to review your information. HOW MUCH WILL THE TOTAL COST BE? The new patient evaluation costs $620.00 in total. This cost includes all shipping costs for your vaginal fluid collection kit, in-house laboratory analysis of your specimens, and the new patient evaluation appointment. If you have health insurance, upon request, FGI will provide you with a Superbill that you can submit to your insurance company.
