At the end of the “Potential Client- Sign Me Up!” registration process, you will be asked to complete two questionnaires,1-2 symptom questionnaires and a general medical questionnaire. The registration will require about 10-12 minutes. You must do it in one session. you CANNOT stop/save/return for completion at a later time.

You will create a Password that will be linked to your email, during the registration process. This will enable you to log onto the “Established Client” tab where you will find “My Chart Questionnaires.” This is the medically secure portal where you can submit medical information to FGI at a tlater time. Since information cannot be ported automatically from one platform to another, it’s necessary to re-enter your demographics. After arriving on “My Chart Questionnaires” proceed to do the following:

1. Complete ONE or TWO max of the Vaginal Symptom Specific Questionnaires.

2. Complete the General Medical  Questionnaire.

Please do so at your soonest convenience; FGI cannot proceed until this information is submitted. It will help you to have the following information handy:

 A) A list of your medical conditions & surgeries

 B) A list of all your medications, dosage, and frequency of use.

You will be notified by phone or email whether the consulting services provided by FGI appear appropriate for your condition. If you are not accepted as a client, then your full registration fee will be reimbursed.
